Personalised diabetes care focused on achieving better blood sugar control and protecting your long-term health. Care includes Type 1 and Type 2 diabetes, prediabetes, gestational diabetes, treatment review and screening for diabetes-related complications.
WHEN TO SEEK CARE
Diabetes may be detected during a routine blood test or may cause symptoms when blood sugar becomes high or low. Early evaluation and regular follow-up can help maintain better control.
If your blood sugar or HbA1c is higher than normal, proper evaluation can confirm diabetes or prediabetes and guide treatment.
Frequent urination, increased thirst, tiredness, unexplained weight loss or blurred vision may be signs of high blood sugar.
If sugar levels remain high or fluctuate despite treatment, your medicines, diet and other possible causes can be reviewed.
Sweating, trembling, dizziness or sudden weakness may occur with low sugar and may require adjustment of medicines or insulin.
Insulin may be required in some people with diabetes. Dose, timing, glucose monitoring and prevention of low sugar can be reviewed.
Regular follow-up helps check blood sugar control, medicines and early signs of diabetes-related health problems.
DIABETES CARE
Diabetes treatment is different for every person. Care is planned according to the type of diabetes, sugar levels, lifestyle, medicines and other health conditions.
Regular insulin treatment, glucose monitoring and dose review help maintain good control and reduce high or low sugar episodes.
Treatment is planned according to your sugar levels and overall health using lifestyle changes, medicines or insulin when needed.
Early lifestyle changes, weight control and regular monitoring can reduce the chance of progressing from prediabetes to diabetes.
Blood sugar during pregnancy is carefully monitored and treated to support the health of both the mother and developing baby.
Persistently high blood sugar or HbA1c is evaluated and the treatment plan is adjusted to improve diabetes control.
Tablets, injections or insulin doses are reviewed according to sugar levels, kidney function and risk of low blood sugar.
Fasting, post-meal and HbA1c results are reviewed together to understand how well diabetes is controlled over time.
LONG-TERM DIABETES CARE
Diabetes can gradually affect different organs even before symptoms become obvious. Regular screening helps identify early changes and allows appropriate treatment when needed.
Blood and urine tests help detect early kidney changes from diabetes, so treatment can begin before problems become advanced.
Regular diabetic eye examination helps detect retinal changes early, sometimes before a person notices problems with vision.
Feet and sensation are checked for numbness, burning, wounds or reduced sensation that may occur due to diabetic nerve problems.
Blood pressure, cholesterol and other risk factors are assessed because diabetes increases the long-term risk of heart attack and stroke.
Body weight and cholesterol are monitored because they can influence blood sugar control as well as future heart health.
Blood sugar, HbA1c, medicines and complication screening results are reviewed together to plan ongoing treatment.
